The screwed pole hanger is also a very common problem we find during our testing of equipment; It happens when a fitting wears out and fails to hold up the fixture it is meant to support.  Instead of being replaced by a new fitting, an inexperienced or untrained person can see no danger in securing the fitting with a screw or bolt.  Not only is this unsafe practice, but, once repaired in this way it can be very hard to detect. As you can see from the small inset picture, from a distance the fitting appears normal. Our trained operators are experienced in spotting problems like this and can report or correct it before a self harm situation can arise.
